MOH ISSUES NEW DIRECTIVES FOR CLEARANCE FROM ENDORSEMENT

The Ministry of Health, Ghana has issued a new directive for clearance form endorsement. After several months of suspending the sale of the clearance forms, the Ministry resumed recently adjusting the price upward to GHS300 from the initial price of GHS 100.

In addition, the Ministry of Health has outlined a directive for endorsing the clearance forms as follows:

1. Present an introductory letter from your current facility, addressed to the Director,

Human Resource for Health Development, (HRHD) of the Ministry of Health.

(NB: Rotational nurses and midwives should submit completion of rotation letter).

2. Attach a passport-size photograph, endorsed by your current facility. (NB: the background must be white and the staff in uniform)

3. Indicate your staff id at the top left or top right corner of the clearance form.

4. Section D (2&3) must be stamped with an official stamp after signature.

5. Indicate your age and marital status on a small piece of paper and attach it to the form.

Endorsement.

6. A Proof of payment (receipt) must be provided when submitting the filled clearance form for endorsement
