REQUIREMENTS #
- A tray containing the following:
- Vaginal medication and applicator
- Perineal Wipes
- Disposable gloves
- Mackintosh and dressing towel
- Receiver for used items
- Treatment Chart (Manual or Electronic)
STEPS #
- Check for the right patient, right medication, right time, right dose against doctor’s order and treatment chart (manual or electronic) as well as the expiry date
- Establish rapport (Refer to steps)
- Explain procedure to patient (Refer to steps)
- Ensure patient’s right to know/consent and to refuse medication
- Perform hand hygiene
- Send tray to bedside
- Encourage patient to void
- Provide privacy
- Assist patient into lithotomy position with the upper leg flexed
- Protect bed with mackintosh and dressing towel at the buttocks
- Fold back the top bedclothes to expose the perineal area
- Perform hand hygiene and wear gloves
- Assess and clean the perineal area with wipes
- Remove gloves and perform hand hygiene
- Wear gloves and unwrap the medication
- Remove the medication and attach to the applicator as per manufacturer’s instructions
- Encourage the patient to relax by breathing through the mouth
- Expose the vaginal orifice with your non-dominant hand
- Insert the applicator gently into the posterior wall of the vagina
- Slowly push the plunger until the applicator is empty
- Remove the applicator and place in a receiver
- Dry perineal area and ask the patient to remain in the supine position for at least ten (10) – fifteen (15) minutes
- Remove gloves and perform hand hygiene
- Dispose off used items and decontaminate applicator and tray
- Document procedure on treatment chart (manual or electronic) and nurses notes
