STEPS #
- Establish rapport (refer steps)
- Explain procedure to client (refer steps)
- Ask the client to empty her bladder and obtain midstream urine
- Provide privacy and perform hand hygiene
- Observe general appearance of client
- Measure height and weight
- Check temperature, pulse respiration and blood pressure
- Assist client to undress and wear gown
- Help client to adopt dorsal position
- Examine scalp and face
- Examine neck, upper limbs and axillae
- Examine breasts and teach client breast self-examination
- Expose and inspect abdomen
- Measure symphysio-fundal height
- Perform fundal and lateral palpation
- Instruct client to bend her knees slightly and breathe slowly through the mouth
- Perform pelvic palpation and assess head descent
- Listen and count foetal heart rate for one minute
- Examine the vulva for any abnormalities
- Instruct client to lie on her side and examine the back
- Examine the lower limbs
- Communicate findings to the client
- Express appreciation to client for her cooperation
- Help her out of bed and assist to re-dress
- Dispose off and decontaminate used items
- Perform hand hygiene
- Record and report findings
